It's your first visit, what should you expect...

Appointments are required for a first-time visit. Your first visit here at Atlas Chiropractic will include a consultation, a chiropractic exam, and a treatment. The first visit will take about 45 min. Following visits will take about 15 min.

Pricing with insurance depends on your specific plan. If you will not be using insurance, please call the office to inquire about pricing. All new patients will need to complete the following printable forms before consultation.

  • Patient Information Form – documents past health history and insurance information to assist the doctor.
  • Pain Analysis Form – document the location where you may be experiencing discomfort or pain on the body.
  • HIPPA Form– gives Atlas Chiropractic consent to disclose health information.
